At My Care Management, we believe health happens at home — and so do we.

We’re a home- and community-based care organization committed to supporting individuals with complex medical and social needs. Our mission is simple: move care out of the facility and into the community where life happens. We specialize in helping Medi-Cal and Medicare-Medicaid members navigate the health system, connect with vital resources, and live safely and independently at home. Our interdisciplinary team of care managers, social workers, and community health workers partner directly with members, families, and providers to ensure seamless coordination across every stage of care. Together, we’re redefining what it means to deliver whole-person care — one home, one neighborhood, and one individual at a time.
