Managing your health can feel overwhelming, especially if you have multiple doctors, medical conditions, medications, or need help finding community resources. MY Care Management, Inc. is here to help.

Our Care Managers work with you, your family, your doctors, specialists, hospitals, and health plan to make sure you receive the care and support you need. Our goal is to help you stay healthy, remain independent, and avoid unnecessary trips to the emergency room or hospital.

Our services may include:

Coordinating appointments with your doctors and specialists Creating a personalized care plan based on your goals Helping you understand your medications and treatment plan Connecting you with food, transportation, housing, and other community resources Supporting you after a hospital or emergency room visit Helping you navigate your health insurance benefits Working closely with your family or caregivers when desired.

All services are provided at no cost to eligible members through participating health plans. Eligibility is determined by your health plan and individual care needs. ECM provides intensive, person-centered care coordination for Medi-Cal members with complex health or social needs as part of California's CalAIM initiative.
